The medical file<\/h2>\n\n\n\n

A medical file is created within the establishment. It contains all of your health information. <\/p>\n\n\n\n

In accordance with the law of March 4, 2002 and the decree of May 21, 2003, the patient, his legal representative or his beneficiaries may obtain communication of this information.<\/p>\n\n\n\n

Your medical file can be consulted on site, or sent directly to your home or to the doctor you have designated.<\/p>\n\n\n\n

Reproduction and shipping fees will apply depending on the terms and conditions.<\/p>\n\n\n\n

The request for written communication of the medical file must be sent directly to management using the required form to: <\/p>\n\n\n\n

Designate your trusted person<\/strong><\/h2>\n\n\n\n

During your stay, you can designate, in writing, a trusted person from your entourage to accompany you throughout your care and decisions to be made. This person, whom the establishment will consider your “trusted person<\/strong>“, will be consulted in the event that you are unable to express your wishes or receive the necessary information for this purpose. They may also, if you wish, attend medical interviews in order to participate in decision-making concerning you. Please note that you can cancel your designation<\/strong> or modify its terms at any time<\/strong>. <\/p>\n\n\n\n

You will be given an information sheet and a form to complete upon admission. This completed and signed document, which will be included in your medical file, should be given to the receptionists in the hospitalization units. <\/p>\n\n\n\n

This designation may be made at any time and is valid for an indefinite period, unless revoked. <\/strong><\/p>\n\n\n\n

Any adult may, if they wish, draw up advance directives, in case, at the end of their life, they are unable to express their wishes<\/strong>. <\/p>\n\n\n\n

These guidelines indicate his wishes regarding the conditions for limiting or stopping treatment.<\/p>\n\n\n\n

They will be consulted prior to the medical decision and their content will prevail over any other non-medical advice. Valid without limitation of duration, they can be canceled or modified at any time. If you wish your instructions to be taken into account, make them accessible to the doctor who will treat you within the establishment: entrust them to him or report their existence and provide the contact details of the person to whom you have entrusted them. <\/p>\n\n\n\n

A form for this purpose is available from health managers. They can also be written on a separate sheet of paper. <\/p>\n\n\n\n

You can download the form which will be given to the staff of the care unit or to the reception secretaries in the hospitalization units.<\/p>\n\n\n\n
